    Essential Duties

    The Intake Clerk, EEG is responsible for the creation of new patient files, and initiating the intake process by obtaining all relevant information about the patient. They will also ensure that all information is accurately entered into the system. 

    General Job Responsibilities

    • Responsible for the intake of all new patient referrals received via email, fax and phone 
    • Initiate the intake process of new patient referrals by creating a new patient file 
    • Ensure referral information including patient demographics, insurance information and office assignment have been entered correctly 
    • Ensure billing sequences are entered correctly for new patient referrals 
    • Analyze, extract and interpret pertinent information from patient medical records, and develop the medical terminology and professional acumen to ensure patient compliance with physician’s orders 
    • Coordinate with referring offices as necessary for complete patient information 
    • Perform other duties as assigned


    General Job Requirements

    • High School Diploma or equivalent required 
    • Minimum one year of patient intake experience preferred
    • Previous experience in using an EMR system preferred 
    • Experience in dealing with third party payor sources preferred
    • Ability to communicate effectively with referral sources, staff, patients and insurance companies 
    • Computer literate and proficient in Microsoft Word and Excel 
    • Strong written and oral communications skills 
    • Ability to multi-task and be highly organized 
    • Able to work independently with minimal supervision 
    • Superior customer service skills 
    • Able to sit for extended periods of time 
    • Repetitive use of fingers and hands
